Also, perception doesn’t affect VATS like it does in 4, just so you aren’t confused. It is needed for some good VATS perks though.
You do have to be careful at point-blank range though. If the target is too close, even 95% hits can miss. Possibly the barrel end is beyond the target.
Y'know I used to wonder the same thing myself... even with excellent condition weapons and "all the right moves" it used to throw off sometimes. Especially with moving, or charging targets... until one day I was chatting with a friend who has served in the military, and seen action, he said to me, and I quote, "It's one thing to nail the targets on a practice range but when it's a real honest-to-God person coming at you, things get a little screwy" - now I just treat it as "reality".
No help I know but just my humble opinion.
