Here the leg is like a peg.

You can click directly on the limb that is crippled, and the doctor's bag will heal that specific limb. in your photo there it appears your head is about 75% more jiggered than your leg. Click on each crippled limb, head, arm, leg, and a doctor's bag will be used for each one.
* If you just click "doctor's bag" though, then it heals every crippled limb only slightly.
And hit that capslock. It will get you running again.
In short, there is no 'bug' here.
I hate to break it to you but I've come across this bug before. It's not a visual bug and no amount of healing and console commands will fix it. The only fix is to reload a save before that instance of crippling.
As a bit of consolation, this bug is EXTREMELY rare. In my 3600 hours of playing New Vegas I've only come across it once or twice, so you just got insanely unlucky. However the bug is not specific to the player, it also affects companions.
