Too many tweaks to the game to mention, you can make survival a real challenge. Read the mod page for all possible changes.
There are many other mods like NMC's Texture pack & Nevada Skies, but they can push your system a bit. So I only listed my 3 favorites since you stated that you want to keep your load order small.

I get that the point of leveling up is to become more powerful but I never liked that you can reach 100 in various skills, as I feel my character deviates from their original purpose.
Not any mod suggestions, but simply a mod idea that I for one would love to see brought to fruition. No longer will we have some crazy god tier pipboy compass, but a handheld one..
