How to leave Vault 22?

After clearing the spores, it tells me to return to Keely on the first floor. The problem is, the door leading to Keely is locked, making it impossible to reach her. Is this a bug?

Did you go to the overseers room to unlock the all of the rooms and did you also get the key inside the common area so you can go to the locked door in the food production area to go to the pest extermination area where Keely is?

I unlocked everything and have the Overseers keycard in my inventory.

Wierd. It should have been opened from the Overseers room.

Oh well you could always open the console, click on the door, type unlock then enter. Should solve your problem.

Wait, there should be an open room just past the data storage room where there are mantises just at the end of the hallway.

Did you go there and is the door closed?

Ran through the quest many times, never a bug. Completed quest at least 3 times. There is something you still need to do but I don't know what.

There's two different ways back to the top. You're probably just trying to go through the locked way that you didn't unlock through the terminal. Turn around and go back the way you came.
