I want to do a new playthrough of NV but I have no idea what I'd do to keep the game fun an fresh, since I've already gotten all endings, and ideas?

Try role-playing with a set of rules. I made a sweet fresh play through by making a barbarian character who's tribe worshipped the grognak barbarian comics (a real life bible to the tribe) and every generation names a new grognak who passes an initiation test and is sent out into the world to destroy monsters like the original grognak.

So my character was a fist fighter with high strength and endurance and low intelligence with the claustrophobic trait (so I had 3 int inside and 5 intelligence outside, getting low intelligence ♥♥♥♥♥♥ dialogue when inside a building).

And the rules I played by is that I had to destroy mutants on sight as they are monsters. And I eat the hearts of my enemies to gain their power (the health replenishing from the cannibal perk when eating a body). And played as a survival of the fittest barbarian , so naturally he sided with the legion. Didn't like technology, and killed brotherhood of steel on sight too (he thought they killed his tribe who used to stow away technology and hide it as it was dangerous and they didn't trust it, and were wiped out by a group of power armoured guys, not necessarily the bos ).

That was one of the funnest plays I have done.

Try and come up with a back story and play to that back story. Get perks that fit that character, regardless of how "useless" that perk is. Play like you're that character and make decisions based on what that character would do if they ran into that situation.

Ps, unarmed is one of the most ridiculously overpowered builds there is. And you get unique moves from npcs and at certain skill levels.

Wow, thats actually a really good idea, thanks! I'm probably gonna try something like this, it might be this, might be something else. Not sure. And also, I know somewhat about the unarmed moves, like the one NCR trooper in with the broken leg.

If you do a few quests for the legion you can get a really good unarmed move and then you can stop helping the legion.

Oh right, I tried to get that in my previous runs, but couldn't, since my unarmed wasn't high enough.

play as a racist and go around killing only people of the opposite race.
Try a pacafist run without cheeseing it by useing companions as weapons

I was sloggin a brew with a friend a couple of weeks ago (Regulator from the Rahr brewery in Fort Worth) when I noticed the cowboy on the label and thought, "there's my next New Vegas character." He only uses revolvers, carbines, knives and fists and does not wear armor of any kind, but I did have to wear Doc Mitchell's Vault 21 suit for a while, which kind of grated. But I got lucky when the Ranger that brought the walkie-talkie from NCR got popped by Vipers or Jackals (I forget which) and left me a cowboy outfit that had no bonuses to complete the get up.

The restrictions I chose have been somewhat of a problem with DM since you have to use Elijah's holorifle until you can get your hands on a knife, a revolver (Police Pistol), and some ammo. I expect that the same will be true when I get around to OWB, but HH and LR shouldn't be as big of a hurdle.

No fast travel and no HUD.

Enjoy! :D

Tried that one a while back. It made for some very interesting mission sequencing but it became a problem when I added a lighting overhaul that made hoofing around at night extremely difficult and Cateye isn't all that easy to come by (I think Gomorrah is about the only reliable supplier) and Nuka-Cola Quartz is even harder to find. Also ran into the "what the heck was I supposed to be doing?" question a lot, but that's a player problem, not a game issue.
