New Vegas Bounties series (for its humor, solid story, and general fun; only install parts one and two when beginning and part three only after completing the first two)
One HUD (I use iHUD, personally, as I could never get this to work, but I suspect the problem is on my end rather than the mod's.)
More Perks (for more opportunities to customize your character)
Those are the ones I'd recommend off the top of my head. If you're looking for graphical enhancements, I'd look at Mojave Sandy Desert, NMC's Texture Pack, and Ojo Bueno for textures; and ENB, Realistic Wasteland Lighting, and Nevada Skies for lighting.
EDIT: I'd also recommend looking at the stickied post for bug fixes, and if you're going to install large amounts of textures and whatnot, definitely get the newest 4GB patch.

Great balance overhaul, that makes enemies a lot less spongy, but also does the same for you. It also affects armor, skills, items, effects, exp and all sorts of things to make the game much more realistic (and therefore, more difficult). I use it in conjunction with PN: Rebalance for a truly masochistic experience.

So many changes, you're better off reading on the Nexus page. It adds many new things to the game, but what makes it truly great is the many things it changes. Bug fixes, inconsistencies, item values, perks and so much more. Really great mod, but its only drawback is that, with so many changes, it can have trouble being compatable with other mods.

Removes quest markers, location markers, and NPC blips from the compass.

Has many different effects that are all changeable/optional, but personally I only use it for the amazing AI changes. No longer will enemies stand there unmoving while you unload into them.

With the NPC health rebalance mods, sneak attacks become super OP. This mod helps mitigate that by lowering the default sneak attack modifier from 4x to 2x (if you download the correct file).
