Here's the "TSL" of the 50 official civs.

There are a total of 35 TSL, and multiple civs can share the same pairs of TSL.

e.g. If England, Spain and Portugal are in the same game, the first civ would start at Veilstone, the second civ would start at Fuego Ironworks, and the last would start at a random location. (First / second / last refers to the order in which the civs are chosen on the set-up screen.)

City states do not have TSL as I thought it could be confusing having the CS's name and its city name differ.

If you don't like the TSL I assign to the civs, you can either turn off TSL at set-up to have random starting locations or edit the Map.xml of this mod yourself to your liking.
