It only changes values in thought defs and situation defs. It won't break anything.
Also: does "below it" mean physically lower in the load order display or with a lower load order number? e.g. if Core is #1, and Grim Reality is #2, then Core can be thought of as numerically "lower" than GR, just not visually per the display
In the load order, the top of the list loads first. So you put HubsLib at the top then any cores for mods you use. This means they load before the mods that require them. Each mod down the list loads after the one above it. The last mod on your list at the bottom is the last to load. If it uses any defs that a mod above it uses, the last mod will win the conflict and it's value will be used. It is possible that the conflict won't be compatible at all and errors will occur, so it's always best to avoid using mods that do the same thing if they don't have a patch.
When people say "put it lower on the list" they mean literally vertically lower. The last mod at the bottom is the lowest on your list. I hope this helps. :)
In the comments, you mentioned that this is only pseudo-compatible with Psychology. Do you know specifically what parts it conflicts with? If it's only the thoughts, then that's perfectly fine with me; I greatly prefer Grim Reality's thought balance. However, I do really love Psychology's social, mental illness, and Kinsey scale systems. I've never suspected that they would conflict, but just in case, will Grim Reality interfere with those at all?
Therapy proved to be trivializing the game for me since bad thoughts basically disappeared absurdly quickly, which took much of their impact away. I'd love Therapy if there was an easy way to directly influence the balance, but there wasn't last time I tried it. I'm using Rim Disorders right now in it's place, but I'm not super happy with it's implementation of it's actual disorders
I don't know if Therapy has any XMLs one could edit, but if so, might adjust it there.
I may have a look when I get time.
I'm not sure how long it'd take for me to make a patch. I'm moving atm and about to start a new job.
But if you know how to basically work with XML might give it a look there meanwhile if I were you.
Check in the files and see which defs are changed. If you can tell me the names of changed defs, I can answer.

In the mod's frontpage you mention it might not be compatible with Psychology, but here you say that it i. Should probably change one or the other to clear any confusions.
Would need others to corroborate.
