Male form not changing is due to a known vanilla bug [forum.paradoxplaza.com] in the title customization window, introduced in v1.18, where only the adjective, but not the title, gets saved.
This mod uses title customization window as the basis for "Edit FoA" window, so unfortunately this vanilla bug manifests as Male form not saving when the FoA is edited.
Hopefully, Paradox will fix this issue soon, in the meantime here is a workaround.
As a workaround, after entering your FoA and saving changes, right click on the "Edit FoA" button in the character/title window and then left-click it, to open Edit FoA window again - at this point the male form you entered previously will be applied; after that exit the Edit FoA window without making changes.
