The icon can be moved by right click and drag, as stated in the description of TM2
There are no options to be shown in the options menu, so why would it show there
Mods can't crash, so I am not sure what you mean by that?
Not appearing in options menu.
Crashes sometimes. (not the game, only the mod).
Does not load last saved on loading map.

Yes I actually have this feature planned and glad to tell you that I also already started working on the update for this mod which is long overdue, just wanna finish NS2 stuff first but rest assured it will be coming sooner rather than later.

For this mod...any chance in the future to adding the option to switch to a whole different theme at once? I find when I'm starting a new map I like to try out different map themes. The problem with that is, I have to keep loading a new game with a map theme, then quit to desktop and back again and again trying them out. I would love to have a one-click switch rather than having to select a different theme item by item. Let me know what ya think.

Options are available after city has loaded.
I very often want different settings per save and resetting it up each time is a real pain :(
Other than that it is amazing.
The easiest and quickest way to mix different themes and textures is, without a doubt, using Theme Mixer.
