It also has a stasis function to only tick world pawns 4 times a day
But a handful of popular mods completely break these 2 functions
Which ones? Is it just the savegame shrinker, or is there something like RuntimeGC that can work in real time and give more precise control over what's removed and when?
theres better newer designed alternatives anyway

I use this Mod to remove all the Filth in my Home Area, and to clear snow. That's really it, because the Storyteller one that cleared the queue when it busted was the other feature I used.
-C:\Program Files (x86)\Steam\steamapps\workshop\content\294100\2945201476
-original DLL, grab the DLL file
-move it to assemblies in the 1.4 file
-Replace file in destination
You only need the dll file
It does make the cleaning of world pawns disabled, but this broke VFE
You disabled the point of the mod
thats why they removed world pawn clearing form the last update

so modders need to effectively decide what hediffs they add are safe to mothball
hence my suggestion earlier was a function that could be told to remove things from world pawns that inhibited vanillas methods since removing hediffs or relations is safer on a save than the hammer gc used to use
Do you know why the game might want to avoid mothballing pawns with certain hediffs like Asthma? I'm not too worried about accurate health simulation of non-leader pawns if it means I can get a bit of performance back.
