Check that you are on the latest version of CK3 and the latest version of the mod. Verify your file integrity.
Orangenord : Seems like a major case of wrong installation. Start from scratch and don't use any other mods. Delete everything, unsuscribe, re suscribe etc
If your save file is from BEFORE the lastest update, then its probably an issue where the index numbers for portraits reference the old index numbers. Paradox added new hair styles (etc...) in the last update... which could have impacted the gene index numbers.
1) Revert to the previous version of CK3 and POD.
or
2) Start a new game and play that (RIP your old save file)
Probably its a good idea to do 2 anyway, as there might be other weirdness with using an old save file on an updated version of the game andmod.

The comparison between New Game Vlad and my Loaded game, he lacks his eye colour, his skin is pale, his hair is now light brown, his face is now more rounded. Whereas for her her entire face structure is just different. It's strange.
A lot of characters are getting portrait DNA for the first time. A few have recently been reworked. I don't think that was done to Vlad Tepes.
