Here's answers to your 3 questions:
1. That object lets you define custom portals as world objects. There's a wiki attached to the Github page (linked in the description) that describes how to work with the advanced features of the mod including these. For normal purposes, though, you don't have to worry about these.
2. Everyone can use any portal, but generally the portals work the most reliably for the person that put them down due to the way the networking works.
3. Everyone needs the mod loaded, including the server if playing on dedicated.
Three more questions please
- what does the PORTAL object do that is black and stands upright?
- If I create a blue and orange portal can my squad mates use them or do they also need their own gun?
- Do they need the mod loaded in their side?
As far as planting a portal 3 clicks away goes, it should be possible, but you'll want to make sure you have the portal gun's max distance setting adjusted appropriately in the addon options. I believe it caps out at 5km since that's the engine's distance limitation for raycasts.
