Unable to do endless run - it was working before

So I'm trying to do endless runs but it just stops at the 3rd boss and I attack the heart. What I don't understand is that I've played endless before and I don't know what's changed. I have ticked the box in custom mode and I'm not sure what else to do.

I am playing with mods so I'm wondering if any of the big mods remove the ability to play endless? I also have infinite spire and replay the spire so maybe they're clashing?

Edit: I've tried messing around a bit and when I load up the game in the setting 'play with mods' with no mods enable, the custom mode modifiers show up on the top right with the respective symbols, like big game hunter etc...
When I load up 'play with mods' even with just 1 mod enabled like base mod or stslib, none of the modifier symbols show up.

Infinite spire prevents regular custom endless from functioning.
To access endless with infinite spire, you'd have to complete the quest that unlocks endless in the quest log.
