Each character has 20 ascension levels (difficulty levels) where each ascension is progressively more difficult than the last.
The 'heart' is an optional boss available at any ascension level if you really want to challenge yourself. You only need to beat Acts 1- 3 to advance ascension levels, you don't have to beat the heart each time.
I would suggest picking the character that you enjoy the most, and then work your way up through the ascension levels for that character. Then pick another and rinse and repeat. Pick whatever is the most fun for you.
You'll learn that there are a lot of ways to play the game, and that no two runs are alike. Just have fun!
(exactly how to get the "true" ending becomes pretty apparent, btw)
Ah, okay. Well, that technically counts as a "win" and a completed run.
There is another "Victory!" ending to find. If you want a hint I can give it to you, but you can probably explore around on your own and find it easily enough.
