Not getting a heart run

I have beaten the game multiple times with all characters but I have never gotten a run with the keys to unlock the heart. Bug or am I missing something?

Just in case you're not missing anything: is there an emblem in the top left corner of the screen, looking angular and three-fold?
If there's not, then there's some actual issue at play here.

is it the just green key you're missing? if you keep avoiding the flaming elite I can understand the confusion. the other two are at least put in your sight at every chest or fire

Also are you doing modded or otherwise altered runs? That might prevent things from triggering.

No mods are used, no emblem in the top left. I cannot obtain any of the three keys. Burning elite does not appear at all, neither does the option in rest site or treasure.

Hrm. Were all your runs done in the same game profile? Each profile has its own unlocks, I think.

When you completed the game with each character, was it on a daily run, or a normal run? Pretty sure you have to beat the game with each character on a normal run, dailies don't count.

Also don't think you can get any keys or challenge the heart on a daily. (I could be wrong, but I am 90% certain of this.)

You need to collect the keys to access the heart. One from campfire (no resting or upgrading) one from elite battle ( the one with the flames) and then one from a treasure (no relic) Once you collected all 3, and beat the 3rd act, you will move on to the finale.

So you don't have heart runs unlocked yet. Check your ingame achievements, do you have the achievements for beating the game with Ironclad, Silent, and Defect completed?

I have only one game profile and achievements for winning with all 4 chars, and yes I have beaten the game in normal runs and with accention... Where can I post for a chance of an admin having a look?

Are you playing with mods? Mods turn achievements off.

You can check your local files. Slay the spire folder -> preferences
Open STSPlayer with a text editor
Check if your wins are registered (set to true):
