I find it OP, and kind of boring. It's super lame cheesing an Elite just so my new run will be artificially off to a stronger start. It's a game design crutch I don't really care for. Also I like all the random start possibilities since it's actually interesting even when you just have to choose the least crappy option.

I'd take the HP, but it's also lame and not very interesting.

As long as you’ve hit the act 1 boss the odds of seeing the max hp/lament choices drop considerably iirc, since that widens up the pool of potential risk free bonuses to choose from. Not sure if there’s a way to remove it entirely but just consistently making it to the act 1 boss makes it so it’ll be rare to see the option in the first place, and for those bad runs that you die early in, you take the health and move on. I know it’s not exactly ‘removing’ it but it’s the best answer I can think of considering I don’t really think there’s a mod out there for it.

Side note though, lament DOES have a pretty massive downside. Usually it requires you to hit mostly/only non-fight spaces until your first elite if you want to cheese it, meaning at most you’ll have only added 2 cards to your deck before that (unless you hit a shop, and then it there’s the issue of picking up cards you might not need just to get over the act 1 aggro hump and not having gold for later). You cheese the elite for an early game relic boost (if it’s even something usable, if you get dark pendant or something like that it’s a bit of a waste) for risking a deck that is going to struggle to beat act 1 unless you really know how to pick your cards (especially on A15+). Could also end up getting 2 combats in the event spaces and not have the momentum for the elite too. Basically comes down to high risk, maybe high reward if you’re lucky. Personally I mostly take it to guarantee the act 1 boss so I get all my options for the next run, not so much that I think the current one will work.

The problem is getting to the Act 1 boss, just to suicide so I can play the run I'd like to play is super lame.

Yea I totally get you, the act 1 boss requirement is pretty much there to prevent rerolling boons by instant quitting, a necessary evil I guess. Personally more a fan of the swap starting for a boss relic lol, if you want potentially stupid advantages lol. Runic dome is free energy if you have enough game knowledge
Lament, OP? It's far from the worst Neow bonus, but it's far from the best either. That 'free' Elite isn't free, the opportunity cost is that you're passing up on an actual permanent bonus to beat an Elite you could've just fought normally. It's often not good for pathing either, you want to get cards from early fights.

Yeah, it's really not OP. It's very much a risk and reward choice.

Yea, that's part of why I hate being forced to take that or some HP. Both are so boring and lame. Someone did give me a really good idea and psuedo-solution: Load a save from the 1st boss at 1HP and just suicide. It'll allow you to always skip the spirits. Not perfect, but a pretty solid solution.
