well the most we actually know about them is that they're a cult that worships a bird deity, though there's some dots you can connect. like the higher ranking cultists become more and more birdlike (from the basic cultists that are just dudes with bird masks to the awakened one). the awakened one is possibly the deity they worship, though it appears to be one of the cultists involved in a ritual that caused them transform into that since it has the ritual dagger lodged in its chest. also the matryoshka relic depicts the awakened one so that's possibly supporting the awakened one being their deity.

for a general one there's no direct confirmation but there's heavy implication that all 4 characters are in a groundhogs day loop created by Neow (the whale at the start of runs) until one of them destroys the spire heart. since every run Neow asks them if they're going to try again and also some events imply it like the council of ghosts event in act 2 where they mention something along the lines of you being one of Neow's "toys"

Once upon a time, there was an event called "Spireblight" - this either caused the Spire to become what it is now, or it made the Spire to appear in first place. Since the Spireblight happened, it looks like the world is rotting, withering, decaying and is becoming deserted. (Source: some relic flavour texts which mention the Spireblight)

The Ironclad made a pact with a Demon to become the strongest of his tribe - this also caused them all, except for him, to die.
The Silent managed to defeat an enemy (Probably something like the Act 3 Nemesis elite) and donned the skull. It appears that she passed this test and therefor was sent to the Spire.
The Defect somehow became broken and was unable to stabilize its system. Neow then helped it to do so. Appearently it was a combat Automaton which was roaming the Spire, probably being a minion - Neow turned it against the Spire.
When the Watcher's tribe recognized the Spire, they decided to send someone (the playable Watcher) there to watch, observe and report.

Neow herself also used to be one of the Spire's "deities" until she got banished - ever since, she sends minoins (the characters) to the heart in order to SLAY THE SPIRE.

to add to the watcher's sensory stone event given the appearance of the time eater and that the watcher is the only one the Time Eater seems to recognize it's likely that the person mentioned in the sensory stone event that stopped reporting became the time eater from being corrupted by the spire or something along those lines

in the Watcher's sensory stone event it's mentioned that she came to the spire when a colleague stopped reporting from there or something along those lines. the time eater wears a robe that's the same color as the watcher's and has similar patterns on it and also when you battle the time eater as the watcher it says something like "I always hated you" therefore it seems likely that the time eater is that person
