How good is "Transform" ?
1. Does different card with different rarity has same chance to be transformed into?
2. Is the transformed Curse counted as "added to deck" and able to trigger Periapt?
3. If a upgraded card is transformed, will the result be upgraded too?
4. If a colorless attack/skill card is transformed, is it possible to obtain an event-only card like Bite or Apparition?
5. Should I always transform Strike and Defend? How good is Transform? How to evaluate its advantage/disadvantage?

1. I vaguely remember hearing about some numbers they tweaked but I might be talking out of my ❤rse right now. If in doubt, assume rarity doesn't matter.
2. Yes. Omamori too.
3. No. Eggs upgrade transformed cards though.
4. I really don't think so. You can't randomly generate them either.

If I may add my five cents, it can either be a big gamble or a rather safe play with little downside, depending on when and how you use it. You can't really rely on it, but it can sometimes be so powerful that it just wins you the game because you get the exact card you needed (for example limit break in a strength deck), or it can trash your deck even further.

It's a good move if you do it on a card you already don't want, or on one of the basic cards. But even then, it never feels good for me to choose the transform card option at the begining of the run, expecially when you get get a bonus that lets you choose a card instead.

I will, pretty much 100% of the time, always transform a card. Most of the time the strike, though sometimes the defend. You pretty much never get a worse outcome.

The only time I won't, is when I'm either already out of them, or I've an imbalance that may get worse with my choice.

Transforming strikes and defends is almost always better than removing one. Unless you get a meme card like hyperbeam or grand finale but chances are relatively low.
