Can someone explain this to me? (SPOILERS!)
I got all 3 characters to the heart.

I don't understand.. I thought this would open after getting the three achievements "Ruby" , "Emerald" , and "Sapphire" but It didn't. So what gives?

In future runs you need to gather the 3 shards by doing specific things to fight the heart on that run. One piece is gained by choosing to "recall" at any campfire, one is gained from any chest but will cause you to skip the relic by taking it, and the last is done by fighting an elite that has a flaming icon on the map. Do all three of these then defeat the act 3 boss and you'll be able to fight the heart.
