After that the keys will appear.
You get one for fighting a flaming elete. You have one chance per act to confront it, so do it when you have confidence your deck can take it.
You find one in a chest, but you must give up the relic in that chest. Again you have at least one chance pr act, so don't give up good relics in act one and two.
The third is found at a bonfire by choosing the recall action. Do it when you don't need to heal or upgrade cards.

Blue Key = Any non-Boss Chest (take instead of relic)
Red Key = Any Campfire aka Recall button
My advice is to pick up the Green Key as soon as you're ready for it.
The other two keys are more situational. Under certain conditions you might not even pick them up if your deck is in no shape or form to defeat the Heart.
I will pick up the Blue Key in Act 1 or 2 only if the relic in the chest is of little value to me. Very often I don't take Dreamcatcher because I rarely Rest because I go for value most times when I visit a Campfire, or I skip Dead Branch if I have tight deck where I don't want any random cards to mess up my game plan - that'll provide you with some examples of how to weigh your options. Furthermore, if I take the Blue Key earlier, I might instead pick up a better relic later on that I otherwise would have had to pass up on if I had to take the key there. Like it can really hurt to pass up on Kunai if you have built your deck around Shuriken when you instead picked up Letter Opener earlier despite your early focus on an Attack-oriented build.
Whenever you visit a Campfire and have max HP or otherwise deem yourself healthy enough and have no critical cards to upgrade, take the Red Key. Another strat is to pick up the key at the last Campfire before the Act 3 Boss - that way you can squeeze the most utility out of your other Campfires to assist with your run. Be careful though, like already stated in this thread, you might take some hits before you reach the last one thus you might not survive the Boss fight unless you choose to Rest and heal up. This is especially true for Ascension 20 with the double Boss fight back to back.
Omg thank you, this is very helpful - I was going by tutorials but I completely missed the bit where you need to get to act 3 with ALL of those 3 characters, I was so confused about what I was doing wrong
