How significant do the choices actually affect the story?
Is it just afew sentences, maybe a full conversation, or are we talking entirely new areas and characters depending on your choices, where you make a different choice and you get still different areas and characters?

Mostly it changes the conversations, but it changes them *a lot* depending on your choices and what skills you have. There's many different ways to resolve quests, and what order you do them in can change things as well. There could be characters and areas on the map you didn't see, depending on your previous choices and how thorough you were or weren't, but it will all be contained on the same map of Martinaise and coast. The vision quest you get on day 4 is mutually exclusive between runs, too.

Sounds interesting, I'll think about it.

Not a ton of new characters or areas unless you chose not to do some very specific things, but if you had played as a regular cop and tried not to bend or break the law in your first run, for instance, it's possible.

Fundamentally different interactions with every character and place. Do they think you are untrustworthy idiot racist? They will treat you differently. Are you lacking in perception? You will not notice the crazy magic hobo in the corner.

Game has 22+ skills that each give you different info and speech options.

That sounds pretty cool.

As others have said it's more about changing how you interact with the story/characters than changing the overall story arc.

So to be clear, depending on the choices I make I will have mutually exclusive and unique conversations with the various characters?

That's the general idea yes, NPC's act differently depending on what type of cop you're playing. I think it's quite an interesting idea as it plays more into the what would my character do than what would I do or even what gives me a +2 longsword!

It's certainly not a game for everyone (no problem with that) but I put it not just in my list of classic CRPG's (even though it is quite different to the average one) but classic games.

It changes the narrative and the knowledge you will get of the investigation or the world. So basically you can be an ♥♥♥♥♥♥♥ and some people will react towards that. The game has let say multiple ending but depends of some decisions you might take during the game, but in the end if you push the main story, does not matter how you are narrating the story you will reach the same ending but with some difference in the narrative.
