Warhammer 40k old ones were at war with necrons, and the warp did not exist before their extinction, warp explosion was caused by eldars after the war in heaven (old ones vs necrons). Warhammer 40k old ones never met chaos gods, because they did not exist yet.
Fantasy old ones were a technological advanced, space faring civilization. They moved between worlds thank to gates a là "stargate". Maybe fantasy world/planet was a laboratory, the old ones tried to create psichic race to balance chaos and end it. But this is not sure. Anyway, the polar gates collapsed, chaos poured into the world and the old ones disappeared.

Now we know nothing of what happened to them. Were they truly wiped out or are they still out there? There are also numerous implications in the Warhammer Lore that the Old Ones are one and the same with the gods of many cultures such as the Elves and the Tomb Kings while the Lizardmen make it clear that their gods are indeed the Old Ones.

No I'm pretty sure that is the Ogres' origin story. They were the last sentient race created by the Old Ones.
