I am going to list all the things I personally did which resulted in being able to play the game.
I had verified integrity of cache (numerous times) and reinstalled multiple times along the way, so I won't mention every time I do.
First:
- I checked my NVIDIA drivers and found a new BIOS version came out 2 days ago for my MOBO, installed both a new GPU driver, and BIOS update.
- Checked Windows Updates, and downloaded the optional Windows 10 version 21H2
- Restarted my PC afterwards to NO effect of launching the game
Second:
- I did a disk cleanup using the Windows “Disk Cleanup” program that comes with windows. Deleted everything it found.
Still NO effect
Third:
I went into Windows 10 “Virus & Threat Protection” and turned off the 'Virus & threat protection settings' which included the 'Cloud Delivered Protection', ' Automatic Sample Submission', 'Tamper Protection'.
(NOTE 1 – the “Real Time Protection” service will turn back on automatically as stated in the description, I had turned it off, but it turned back on sometime afterwards)
(NOTE 2 – I have always had “Windows Firewall” turned off from the moment I fresh install any Windows OS, take this into consideration as well)
Fourth :
I 'right clicked properties' on “Insurgency Sandstorm” under the Steam Library list, and in the 'General' section I unchecked 'Enable the Steam Overlay while in-game' and 'Use Desktop Game Theatre while SteamVR is active'
At this point I did not verify if the game would launch exclusively by changing these settings in windows protection services.
Fifth: I started uninstalling programs in the “Control Panel” under 'Programs and Features'.
Here is a list of the programs I had uninstalled.
Software:
-Apple Application Support
-Apple Software Update
-CoreTemp
-CPU-Z
-Hwmonitor (CPUID)
-Dropbox
-Dropbox Update Helper
-Malware Bytes
-Microsoft Edge Updater (Edge is disabled)
-HWmonitor
-Plex Media Server
-PS Remote Play
Drivers:
-SoundBlaster Z-series control panel/software (for my PCIe soundcard)
(I removed my Soundblaster ZxR PCIe soundcard at the same time)
Game Services:
-Epic Games Launcher
-Bethesda Game Launcher
-Rockstar Game Launcher
-Ubisoft Connect
Please keep in mind this worked for ME. It may not work for you, however I strongly believe this worked solely after uninstalling something. Check for programs that would have a hardware monitor or overlay, as Easy Anti Cheat is probably the reason for not launching despite not giving a proper error.

Thanks for attempting to help, but unfortunately none of these suggestions did the trick either. I have tried everything I can imagine other than clean installing windows and that's not worth it for me. I guess I'll wait for a hot fix or update to potentially solve my issue or just stop playing entirely.
sorry it was no good. Something tells me the EAC doesn't like certain files/software, so if there is anything you can possibly think of to get rid of try removing it, even if its temporary. Because I understand not wanting to reinstall windows, its a pain. Best of Luck

Ay I found the fix for this error code, this code is usually corrupted game files
Error occurs in that they changed the pak game files names, so either yours didn't automatically change somehow whenever they made that change, or you were trying to reinstall of of an external hard drive like I was to reinstall at 100mb/s
The fix is:
Just do a fresh install straight from steam
Or if you don't wanna wait for it to download go to
files - the install drive>steam>steamapps>common>sandstorm>Insurgency>Content>Paks
See the old naming scheme which was for example on the first pak file "pakchunk0-WindowsClient_0_P.pak" and then rename it to the new naming scheme which for that file would be "pakchunk0-WindowsClient.pak", and just go through all the old files and switch it to the new scheme, recently they added pakchunk23, 24, and 25
So then copy the >steam>steamapps>common>sandstorm folder with nothing else in it but the renamed paks with >sandstorm>Insurgency>Content>Paks then stick it into the steam install folder
>steam>steamapps>common then stick the copied insurgency folder with the renamed paks and hit install on steam, it'll look for existing game files and save you a bunch of time and auto install those 3 new pak files, or if this makes zero sense then just do a fresh install haha
You also gotta go to Insurgency>Plugins folder and delete "OnlineSubsystemEOS" they also got rid of that and it might cause some issues, idk for sure tho

A Serial size mismatch error typically means that your game installation is corrupted. This can be resolved in a number of ways, but the ones I find that work the most often are:
1) Verifying the integrity of the game file.
2) Perform a clean install of the game through Steam.
3) Reinstall the game on a different drive if the options above fail.
