It's really inconsistent.
They are exaggerating the effects of having a PEGI 18 game. the equivalent of 18 rating has bad effects in other places such as ESRB's AO rating, but a PEGI 18 rating isn't really that big of a deal.

The English release of Bandai Namco's Tales Of Berseria has a translation issue in one of the story recap journals. The in-game journal accurately describes the Japanese sword stabbing scene, despite this being changed in English versions
If you're going to censor something, at least put in the extra effort to make it consistent. It helps to at least provide a consistent story. The fact that they only changed one either shows that they are just lazy or flat out just didn't care.
Blame the authority that decides what's acceptable and what isn't.
And speaking of censorship, take a good look at Velvet's profile background picture that you can buy on Steam. Valve isn't a saint. They are protecting the masses from tainting their eyes with the figure of the FRICKING MAIN CHARACTER!

the US is more OK with Violence, but Sexual content is heavily viewed as wrong(ever see an NC-17 movies play in any movie theaters? and remember the X-box title Guy game? that was removed from game stores because one of the girls featured in the game was 17 at the time).
in Japan, Violence is frowned on(there is a reason for it too) in Video games, but having characters under 18 in lewd poses or such is more accepted(although with limits).
sure you can argue that the artists should have their original works respected, but you can't expect everyone to view it the same way. some may call a game like Gal-gun Double Peace a great rail shooter, but others will view it simpily as a game for perverts. and while some may say, well that's who they should aim it at then, the fact is, that may mean they dont make enough to cover the cost of the game. you look at it from a business perspective, and you can see some of the reasons for it.
and yes I bought it from the JP store at ¥2000 on sale and own it on steam too, just for support.
