Report a translation problem
The settlers and npcs spawn 'somewhere above the ground' when you load the cell that they're supposed to populate, which is why sometimes you'll find brahmin on the roof or falling out of the sky.
If the area you're putting things on has been modified at all, the game just doesn't really know what to do with it. They will still count toward whatever numbers you have need for if they're settlers - as long as they show green as doing their function, they're actually doing it.
If you sleep a little or come back by walking, generally they should reappear in their spots, but again if you've modified or build anything, all that goes out the window. With that much stuff built over the ground, they are being spawned inside those blocks, because the game doesn't actually know they're there.
You also have to consider you kinda caused it, lol. When a settlement loads it loads certain items in certain orders. So if the NPCs load before the floors do....you get the idea. So some of it comes from overbuilding, and wlel you are definitely WAY over budget.
I have found smaller settlements of 20-30 to be far more stable and you see less issues like that.
Now the find funny thing is that sometimes you can place a stair inside a floor using Place Everywhere and the mob can sometimes climb out. This is because a navmesh for stairs usually starts an animation and animations ignore physics.
I placed a pink square navmesh on the block area where the caretaker got stuck after I got him out of the hole

Owe I sometimes don't use the foundation blocks and just let them find there own way out, some mods now have wall's that slide open to get through them, Ethreon's castle in the sky has wall doors and so does Snappy HouseKit by robboten mod
At the The Slog it;s funny seeing the caravan guards heads sticking out of the ground, because there are invisible ground sitting markers at the caravan spot there
I'm yet to see if 5like Thematic and Practical - Workshop Settlements - Structures and Furnitures also known for Caravan Trading Post - Enhanced mod if the caravan individual stands well work at the slog?
I fixed this by placing a brick floor where the faulty piece was and pushing it down so it was just under the faulty piece.
Another possibility is that the game is picking up the terrain below the piece and not the block. But I think it is load issue.
I think most of what I used out of that mod was the bunk beds because they were two separate beds so there was no assignment issues with them. Also had a bunch of nifty beds in it.
I read it just fine apparently you did not as he asked for a mod not info on the engine. I respond to the OP not to random posters, except in this case.

No worries, I would never need help from you, my first computer was in 1977, doubt if you were ever born yet. I have helped so many in the past ten years on Steam I don't need you on my list since you know it all.... in fact I'll just add you to my black list, don't need your advice.
And in over 3 play throughs, I've only had NPCs fall through/into the ground a couple of times, so yes it is related to his mods.
Followed by "I been here forever and do not need help from anyone." Which hate to break it to you EVERYONE needs help with something at some point.
