Permadeath is gone?
I quit the game in the past because the devs ruined survival mode by allowing you to cheat whenever you want. My mind is like, 'why am I struggling for this if I can just get it gifted to me by changing one setting? Now I come to play permadeath and see the only mode I would play because I can't be urged to cheat is gone. Amazing how my favorite game quickly became my most hated.

Permadeath is still there, it's a difficulty setting that you have to turn on.
it's permanent once toggled on.

Make a new game, select custom game, then select the Permadeath preset.

Do I still share the same world as the cheaters, though?

If I have pvp on to add to the danger I don't want to run into someone who didn't work for anything they have and has zero risk, so there is no sense of caution

More then likely, yes. The game can be freely modded so you could verly likely run into cheaters. I would defilitely turn off multiplayer while playing in Permadeath mode.

Game Vet glasses make it seem that way after playing hundreds of hours. I remember starting out - was pretty confused at times.

cheaters? no matter what playthrough style you choose there will be others inhabiting that same server/universe/whatever that may do things with their save and playthrough that go against the devs intents, even things like refiner duping. There's not much any of us can do about it and constant worrying about others or comparison will just eat at you and you'll begin to convince yourself that it's ruining your experience when it does nothing of the sort. Best of luck

You aren't wrong. I just have issues with stuff like this. Having such a generous difficulty slider in itself pulls me out of the immersion. I don't know why. I liked how the game had set difficulties back in the day. Back then I loved this game to pieces. I felt so immersed. I felt like I was on a lonely adventure in infinite space. It gave me amazing feelings. I know I am picky about something like this. I wish so much I could move past this. Part of being in the world struggling with others is part of my immersion. If I know that many are treating it like creative mode it just annoys my mind. I know I will likely never see them in any meaningful way, but yet I still can't move past this. I wish I could. A doctor told me I am ocd, so whether there is a connection there, I do not know.

There is no difference between how it is now and how it was back then, if you play Permadeath. Permadeath players are unable to set things lower than the default permadeath settings.

The only thing that really changed, for Permadeath, is that some of the settings can be set higher then even permadeath defaults, which wasn't the case prior to custom difficulty options.
Permadeath players are isolated from all other players, so you will not see anyone who is playing in a mode where the settings can be lowered.

Modding and save editing is still an issue, but it was back then too.

if you only change the setting for Death Consequences then the game should recognise the save as Permadeath rather than Custom... (it has a skull beside the save file, name of bases, etc.)

Also worth noting that there are many ways to "cheat" in NMS that have literally nothing to do with your chosen game mode, so not sure what you are referring to by "cheaters"...

But you should only see Permadeath players if you've got an actual Permadeath save... Either way, I would always recommend to turn PvP off...

Yup, only threat I've had so far was a bugged pirate ship that only took shield damage, but no hull damage... I simply ran away once it was obvious that destroying it was impossible and I started taking damage to tech...

You can change any of the settings to higher(harder) than the Permadeath defaults and the game will still recognize it as permadeath,. You cannot change any of them lower if you select permadeath in the first place.
