I don't get it. What exactly are you supposed to do? I drank an elixir of glass. That's all I've got.

I don't get it. I don't get a scanner signal for it. I assume it's any planet in this system. Do you enter the final portal first? I thought you needed the water elixir for that.

Breach 5 milestone was before it, so yes, you do need to take the final portal.

You'll then get a marker for the approximate location of the Boundary collapse which will lead you to the gravesite.

Then this thing is bugged because I'm at the Breach 5 portal and have every blood elixir and I keep getting that standard "enter portal address" prompt.

Have a look for another marked portal on one of the other planets in the system.
I've done Exp16 3 times, and on a couple of occasions, using the quicksilver potion marked a different planet portal, but the one I used to arrive was still marked on my HUD so I went to that one 1st. Got the same "standard portal" response from the incorrect portal even with the right mission selected and the right blood elixir ready and as soon as I went to the other one, it worked fine.
