Because you have already been to the system you wish to return to, you can find the glyph address for it by going into your Discoveries and adding the planet to your Wonders. THen view your wonders and the glyph sequence for that planet will show in the mouseover tooltip.
Memorize it (or write it down) and then return to a portal in the system you are in, fix the portal, and then input those glyphs (in the order which they showed in your wonders). The newly activated portal will then take you to that desired planet.
If you visited the space station in the system you're trying to return to, then use a teleporter to get back there...
I always visit space stations before leaving a system, and if it's an uninhabited system (no station) that I know I'll want to return to, I build a base so I can teleport to it
The location of the portal is limited to this expedition, but you can discover it by creating and drinking a elixir of silver.
This is probably the address you should enter.

I just thought it was worth mentioning that bases function as teleport locations even without a teleporter built there, and are worth building if there is no Space Station (assuming you want to get back there)
