Did you die on standard difficulty much on GOG?
How are you dying now on steam?
I think how to prevent that is the real question. I know things happen but you seem to imply that it happens fairly often (?).
That sounds like a bug or glitch. The game does glitch out on occasion and all is better with a reload. I define a bug as constant. It does not go away on a reload. Not sure it is worth dying to test it.
The game does get confused in MP at times. If you are playing near someone in creative mode, it may be co-mingling the two modes. You get a grave, but no items transfer to it because it is trying to sync with your auto-group.
As far as dying, I've tried most different ways. Falling from height, hazardous flora, drowning etc. I always drop a grave but I never actually lose any items. Survival Death Consequences work properly (deleting my inventory), but I can't for the life of me get the Standard Death Consequences option to work properly.
100% success rate replicating this. The setting worked properly with GoG, but stopped working properly a day or two ago. This actually prompted me to get it on Steam to see if it would be the same issue here, and it is.
If it was a rogue config somewhere you'd think the clean reinstall would get it, but here I am. Googled for hours, no results. Nobody having a similar issue to me. Looking at the responses it looks like that's the case here too.
