There is a limited number of settlement slots that show the buffs you have received. From what I can tell, the excess do get applied to your settlement, they just aren't "public list" worthy.
As you follow it, some visitor gifts do supersede existing buffs. While not clear, it seems that those old ones just disappear and are no longer active.
Makes those particular results for the settlement timed-event entirely pointless because, in most cases, those "gifts" are the only thing involved....so you basically just waste your time going to the settlement to respond for nothing.
I always am thinking they'll fix it someday, but so far...nope.
Also, crazy how many of the replies you got when this thread was first made (a year ago); yet, literally, not one understood what you were even talking about.
The 'gift' of items (rather than a settlement specific reward/buff or bane) is a random possibility of what a visitor interaction at the settlement can possibly be.
There are quite a few different ones, but they are not particularly common as compared to the various visitors and events for the settlement itself. Possible gifts include, but are not limited to, Hadal cores, storm crystals, hypnotic eyes and even just a direct nanite gift.
Of course, as indicated above, you don't actual receive anything. Nor does it give the settlement any kind of bonus or loss. Making your choice just simply does nothing at all.
But I have never seen the described gifts DELIVERED, either. Not to any "Town Inventory", nor to my own Inventory.
