Most of the rest of your list I've taken damage from but not died due to. I'd like to read the flavor texts from the traveller's grave sites but I discovered in loop 1 that wasn't a good idea. I just click through it as fast as I can and enter my ship that I pre-summoned.
Edit - If pirates are attacking you as you enter the station and you exit your ship there is a bug that will allow you to continue to take damage from the pirates. Since exiting the star ship creates an auto save you should immediately reload that autosave since that will clear the pirate attack.
I buy that excuse. I made it through the whole expedition with only the required death and the one at the end but almost every one of my close calls in the early part of the expedition were caused by exactly that same thing. That and my tendency to somehow land in hazardous plants any time I use the jetpack to 'escape' other dangers.
I thought using ESC was like a pause?? In single player mode have pressed ESC and gone for breaks without any problems or dying.......
Any comments on what killed me?
Pirates attacking you when parked in your ship on the ground can happen, and they can blow you up.
As for escape, if you have multiplayer enabled in network options it does not pause the game.
