Now six years later they still have a ♥♥♥♥♥♥ unstable game on their hands, multiplayer stability is laughable at best and you have nonsensical stuff like items getting deleted without any warning .
Reloading a previous save: I spent ages buying & scrapping ships for upgrades & nanites so no, not doing that.

I understand that some parts of multitool handling aren't explained well, but this is ridiculous. You clicked on a button that reads "Exchange" (instead of "Claim") and had the text "Trade in current multitool" right below. You literally just have to understand basic English to become aware that your current multitool would be gone afterwards, What part of "trade in" was difficult to understand for you?
But anyway. Even if there was a bug, you could just reload to get your old multitool back. Nanites and upgrades are easy to get again.

If it's not there you can temporarily reveal it and any others you may have missed by exiting the game, launching the game again and loading (or starting) a Creative mode save.

You can test it yourself.
Look, there _is_ of course a small chance that you ran into a bug that no one else has reported within several years (people sometimes do report that their multitool is gone, but they usually just don't know where to find the old one, as that information is somewhat hidden). It's not impossible.
But unless there's some way to reproduce this, it seems much more likely that you either
a) misclicked on the "Exchange" button, or
b) weren't fully focused on the game and habitually clicked on the leftmost green button without noticing that it read "Exchange" (because there no longer was a "Buy" button after you acquired 3 multitools), or
c) actually still have the old multitool in your collection (I suppose you'd have found it by now, but if you're very sure about seeing and clicking a "Buy" button, then you may want to double-check your tool collection).
If you find a way to reproduce the situation that you're reporting (i.e. a "Buy" button appearing while already having 3 multitools, and your current multitool being replaced after clicking it), please tell us how. I spent about 30 minutes trying to do it, and could not. I always only saw the "Buy" button when I had room for another multitool.
1. Open the "Quick Menu"
2. Scroll to the far left and select "Utilities"
3. In the submenu, scroll to the far left and select "Swap Multitool"
4. You should now see 1-3 multitools that you can scroll through and select. Depending on the contrast between the tool color and the background they may be hard to see.
How many multitools do you have in that list?
You need to set shortcuts for each MT via CTRL 1 to 6 and then switch by just pressing keys 1-6, as in other games with weapon selection. Easy...
