2. I am not sure, not played for ages, teleporting and warp gates (the big star gate looking things) will get you where you need to be. also black holes, although can be unpredictable.
Select a target system from your Discoveries Timeline then fly out and check the distance using the galaxy map. If it's not close enough for your liking you can fly back, teleport to another station and look again.
You go into the Station and open the Teleporter menu
If you want to make sure it is on your list, you can Teleport back and forth from some places. That will definitely make sure it is recorded.
Also, 50 systems is the limit you can have listed?! I thought people had a hundred or more discoveries on record! That's absolutely abysmal! The interface is awkward enough when the list gets large, let alone having something like that to discourage things even more. Sure, it ultimately doesn't matter in the extreme long run, but while in a given area, it really does matter a lot.
Teleporting does kind of feel like warping, and I guess the terms can get mixed up.
I'm up to 74 so far and haven't had any drop from the list due to neglect that I can tell. I suppose that there's a system I can't remember because I've only visited it once which might have dropped off but I think that all of the systems in my current active save (Euclid) are still there.
Just warping through a system isn't enough to add it to the teleporter, even if one visits a planet or two. Landing in the space station should do it, though.

The first is a sure one, for the second I need to re-confirm still.
