1. No, your base is deleted and you need to build a new one in your new galaxy.

Personally I'm still split on whether I want to delete my awesome underwater base and rebuild it, or just stay in Euclid Galaxy and meet people.
Kind of sucks.
2. You can still play with your friends, but when you load your game, you will appear in the second galaxy. If they join your game they will travel to your galaxy, and if you join theirs you will travel to the first galaxy (Euclid).
3. You don't lose anything, when you arrive in the second galaxy most of your equipment and ship is broken and you have to repair.
EDIT: Don't try to teleport to the second galaxy if you joined a game in the first, you will appear in the middle of the space and will die by cosmic rays, losing all your equipment. The same when you start your game in the second galaxy, and try to teleport to the first. Only travel in the galaxy you are.
Same here...do not try teleporting home from a station...cosmic radiation death!
Did not try just jumping out of my ship and quitting...that might work to get back to your home galaxy.
Yes, for multiplayer, everyone will end up in the same system and galaxy where the game session that they join is taking place.
Only the materials used to build your base (but not stuff inside containers) if you don't dismantle it first.
All tech in your main inventory, multi-tool, and the main inventory of your currently active ship will be broken and need to be repaired with materials, but that's not really 'losing' the upgrades exactly.
Tech installed in the tech slots will not break.
The freighter goes with you, as well as the frigates, and any other non-active ships that you have, and the tech on those will be intact.
Also your teleporter list will be empty and need to be repopulated with stuff in the new galaxy. They don't work across galaxies. You can't switch galaxies with portal addresses, either.
I've done that twice, both times returning back to a place surrounded by stars with the word "Contact". It feels great owning a star (if you can find a decent unclaimed one around the < 5K LY core). An other galaxy I visited was just empty, perhaps I should've gone to that core too but no doubt it would also be empty as people there would tend to move on I would imagine.
