You need to hunt down glyphs left by traveler graves. There are 16 different glyphs. Then you need to fuel up an ancient portal you can find by visiting relic sites and put the glyph code in like a stargate.

There is a site I will post. It has a glyph/coordinate calculator. If you type in the coordinates, it will give you the glyphs needed to get to the planet via portals. Here is the address......

I am somewhat confused.
Is there a point to having the Hexadecimal address in numbers? I understand that the Portal (at the Anomaly?) will take Glyphs for me to be warped somewhere. WIll it also take the Hexadecimal address? If not, then what's the benefit of having it?
I don't have any of the Glyphs so far. Hence I guess I am getting ahead of myself (?)

yes, getting a little ahead of yourself..and only the Glyphs operate the portals

The hex coords are what your signal booster gives. Portals work on glyphs. The decoder I linked you to converts the hex numbers to portal coords. So, if you find a nice planet, and want to keep track of it, open your signal booster. Take a screenshot of it showing the hex address. Anytime later you can plug them into the decoder and use a portal to revisit the planet even from across the galaxy. The point is this, the portal creator used glyphs, a different language of sorts. We as travelers use coords. So, we can decode the hex address to use a portal.
