Maybe once you leave space station into space you must scan to get that waypoint. By always following the onscreen instructions (instead of simply reading what's in the log) you can never fail any mission, believe me. And if in doubt: reactivate the mission and read onscreen after leaving the log menu.
No, my point 2 is correct. I am literally following the on-screen instructions and am constantly activating and reactivating that particular mission in the log. The onscreen instructions are literally sending me to the teleporter on the space station in my home system. I have tried scanning the system after leaving the station, have tried scanning from my ship outside my base in that system, and in my galactic map there is only one system, my home system, with that icon for that quest.
So yes, it is definitely possible to not be able to continue this mission even when following the on-screen instructions.
- Teleport in the system with the mission icon;
- Go to space and look for the mission icon; if not there, then scan.
- If still no go, re-activate mission in the log and follow instructions.
And this doesn't work? c'mon...
If really it doesn't work, then something went terribly wrong. Go through a black hole and reset the mission, this will spawn new waypoints. I did this mission 3 times already in 3 different starts, impossible to do it wrong. The entire storyline is crystal clear, my dog could do it...
I also haven't reached a black hole yet so going through a black hole isn't currently an option. Nor should it be if the mission wasn't bugged, right?
Go to space anomaly, go to Polo, ask for black hole, go through black hole, reset the mission. Do it in that current system.
I do have a guess how I'm in this situation - when I went through the breach to visit Atlas and he dumped me on the planet where you meet Null to make the Mind-Arc, I used the Space Anomoly to leave that system rather than use that system's space station or my ship to warp out. That means though that that particular system was never locked in the list of places I can teleport back to. If this mission is meant to teleport me back to THAT system then it's broken because it's instead trying to send me to my original home system now.
Of course that's just a guess, I have no idea if the mission is intended to have me teleport back to the planet I got dumped on when I went through the breach to Atlas. But it is I believe the only system that I didn't either use a space station in or build a base with a teleporter on to teleport back to it.
Getting into the situation is quite easy, as the player has just been told the anomaly will teleport them home from anywhere, some players try exactly that, leaving Artemis half a galaxy away with no way to return. Terrible quest design.
Here's a thread discussing solutions. It was easiest for me to reload a save and then not leave the system, but there are other solutions.

As far as how it happens, I think Bob's post above is essentially it. I used the Anomoly to teleport back to my home base and had no way to get back to where it expected me to return. Really what the quest should do is simply send you to ANY holographic communicator once you have the arc, not that particular one in that particular system. That would resolve the bug.
In your log you chose the mission and use the middle mouse button, keep it pressed. This will reset the mission and spawn new waypoints in the current system. At least it should.
I'm playing in VR--using the Valve Index--so no mouse usage. Just before closing down the game for tonight, I went through a black hole and just landed on a planet so I could save. When I log back in tomorrow, I'll see if I can load up my game without VR--even though I've previously been playing exclusively in VR--just to see if the mouse reset works.
