when you run too far away, Ruvik could teleport next to you and instant kill you. so the point is that you must kite him around, but dont run too far away from him.
I tried hiding, but killed anyway (probably he saw me hiding).
also, if you stopped moving, he could teleport beside you as well.
to answer your question, he will randomly spawn ONCE per area. lets assume he did appear and you manage to escape from him, then when the scene is over, he WONT appear again on that area. so make sure you save often.
as for me, I always dieded when he appears,
so what I do is rushing to brain puzzle fast (will autosave), and then run back to the machine gate (small cutscene) and then into the mirror room. if he appears, I dieded, and reload....

I hated this part too, but I found that using the staircases in the Main Hall was the best way to put some distance between you & Ruvik. Just always make sure you're keeping an eye where you're going because he does have a tendancy to pop up in front of you if you get too far away.

I keep my eyes on him, I jog ahead a bit and any time he runs I run. That's pretty reliable for me. Rarely he'll spawn as you open a door then kill you without you having much chance, that sucks.
He's on a timer from what I've noticed, something like 10 minutes between appearances, if you move quick you'll only see him once or twice in the level. I've heard his spawns are limited as well. If he is on a timer then that might mean save points are a bad idea.
I don't think there's much in the game that is able to match trying to deal with two haunted ambushing you when suddenly the screen turns blue and you have to figure out where Ruvik's coming from before your head explodes.
