In my previous house (a small 1 room non-burnt cabin I fortified extensively), I successfully ignored many screamers, going about my business inside. I was a bit paranoid at first but realized that merely being "sensed" did not trigger their summon- they just die on spikes.
There may have been a small spot that I had line of sight to the ground, and that screamer took just the right approach at the right moment to catch a glimpse of me. The next one died on spikes without summoning.
I would like to see screamers spawn infrequently in large derelict towns. Seeing them do their thing and getting a chain reaction of screamers summoning screamers can get quite interesting! ;)
Zombies will go into "Sensed" state if you make any non-crafting noise while they're within 5 so so blocks of you. That includes open boxes and chests, searching, checking your forge... ANYTHING that generates sound in the game. Even placing Wooden Frames.
About the only thing you can do while hidden (and remain hidden) is use your personal crafting menu.
Any time a screamer screams it spawns a mini horde around you. Horde size is entirely dependent on your game settings.

Also, a couple of other things to consider: certain actions make more "noise" depending upon whether your toon is crouching (sneaking.);
transferring loot items individually to inventory makes less "noise" than hitting "Loot All" button.
You could always add a screamer or two to each spawn group that you are likely to find in towns/cities/wasteland (that way, they'll summon their own horde if and when they spot your toon).
This could provide some control (don't get spotted ...no extra horde.)
Increase the chance for a random horde to spawn, or the chance for individual zed types (so e.g. add
<entity name="zombieScreamer" prob="1" />
to the Entitygroup (whichever group you want)

EDITED EDIT: You could also add screamers to the Day7 & 14 hordes (which, lets face it, can be pretty weak) particularly after the day 49 horde has been dealt with.
I'm chipping in. :D Anyway one thing to mention is that I've found zombies tend to magically sense you if they're really close to you when you're in a building (On the other side of the wall within a block or two) even if you are crouched and doing literally nothing.
That's more for other people looking in the topic for tips though. :3 Make sure you guys don't hug outside walls okay?
