Difficulty setting, presumably. Or you had higher game stage than you should have, or you were in the wasteland or something. Under normal circumstances you shouldn't see screamers on the first night. You'd have to go out of your way and light like 50 camp fires to make a screamer spawn that early.
On insane, you can get ferals and dire wolves on the first night. It's rare but it happens. Sneaking usually makes them easy to avoid unless you've also got the game set to feral senses.
All of these things are possible, but only if you go out of your way to set up the game to make them possible.
Difficulty set to Adventurer (Default)
I lit my campfire for about 30 seconds to boil a couple of jars of water.
Location, uh.... There was a desert nearby. As in, visible from where I was, but would still take an hour or two (in game time) to walk over to it. Otherwise, it was a pretty standard, green area, with trees and stuff.
All my experience settings are on their defaults.
Same settings, I had a Dire Wolf ambush me from a mechanic building, two doors down from the trader, on day 3, the previous map.
Rads are pretty damn OP early on. I ended up dead in the ceilling again as a tourist went full aggro from the rooftop, he went all the way around the rooftop and dropped down the ladder and fell on my head when i was low in health.

You know zeds can climb ladders, right? Even if the giant black dog didn't get up the ladders, others would've.
Dire Wolves, on the other hand, have considerably more health and do substantially more damage. So, a little stone spear isn't going to do much.
The last Dire Wolf I faced, on my previous map, I unloaded over half a clip into it, with a pipe rifle, before it went down.
I just... I don't know where these guys came from. I fully expected to have to poke a regular zombie or two, on the stairs, because I had an open door and window. I had next to nothing to fix/block those. But, a couple of regular zombies ambling up the stairs wouldn't have been an issue. But, what came at me was something I would've thought to have to deal with post-level 12, or something. I don't know. Not at level 2, on the first night.
I don't know if there is a bug that made them spawn early, or if I got a REALLY crap RNG roll. I don't know if it's because I was in a town (on the edge of a town).
I lost nothing. My bag of stuff was still outside, and not going anywhere soon. I wasn't over level 6 so I didn't have the exp debuff. I just kept respawning and bare fisting them until they FINALLY died. If anything, it made the night go faster because I wasn't just sitting there, twiddling my thumbs waiting for daylight.
It's just baffling that this happened.
There's your problem. The stairs should have been removed or blocked. Try again.
Yes, yes, the stairs. The stairs. Let's all just ignore the fact that there was FOUR very powerful zombies on my doorstep on the FIRST NIGHT.
Not ignoring anything. Simply stating that you could have easily made it much harder for them to reach you. That is all.
