To obtain one of these, click the 'SteamVR' menu, 'Create System Report' and then 'Copy to Clipboard' or 'Save to File' and post the results somewhere. (There'll be a fair amount of detail so it won't fit in a comment here. Google Drive, Dropbox and similar seem to be good solutions.)

I'll also note that, between then and making this, I updated my GPU drivers. I don't know if that's done anything, but I figured it might be worth mentioning, at any rate.
Looks like your Vive trackers are losing connection shortly before the Rift does - erm ... Vive trackers? You have quite the setup!
Everything is attached to the same USB controller - with three Rift cameras, the Rift itself and the three Vive tracker dongles, I wouldn't be surprised if USB is getting overloaded.
I'd try simplifying things a bit and working up from there.
