Headset Display Disconnected - "Unknown USB device (set address failed)" - Valve Index - DOA
I have followed all faq, reset headset, etc, it appears I am seeing possibly a hardware or driver failure before getting into SteamVR at all.

I filed a ticket HT-MPN8-MDHF-XFRK Saturday with no response yet. I just PM'd the valve support reddit user, and am posting here hoping for some help.

I have removed all extraneous USB devices with the one exception being my wireless receiver for the keyboard. Upon plugging in the usb (in any usb port, including the 2080ti via usb-c adapter) I get the same message:

> WinUsb device
> This device cannot start. (Code 10)
> A device which does not exist was specified.

In most cases, above it in device manager I have also seen:

> Unknown USB device (set address failed)
> Windows has stopped this device because it has reported problems. (Code 43)
> The USB set address request failed.

In my ticket I have posted the system report as well as screenshots. I can provide anything needed to troubleshoot this situation. Please help if you have any suggestions

If you are getting error 208 "Headset Display Disconnected" then all of those USB errors are unlikely to be causing you actual problems. In order to get error 208, the important parts of the USB must be working.

I would recommend focusing on error 208, including checking the DP connection at the PC (make sure it's seated, make sure it's in your primary GPU), upgrading video card drivers, checking the tether connection at the HMD (be careful with this one so you don't scratch the lenses).

Hey @bendotcom - I tried all the connections reseating, drivers, os patches etc - nothing.

Confirmed 208 error.

You mentioned display port - that was it! I recall reading something about different rendering pipelines on the 2080ti (hardware-wise), and my HDMI for my monitor must require me to be in a different displayport. I moved it and connected to the headset! Thank you!

I still have the WinUsb error in device manager originating from the headset - not sure what to do about that.

Is there anything New? I have this winusb error as well but hmd works? But i want to resolve this error...
