Report a translation problem
This is a problem with the pairing in SteamVR itself.
The Bluetooth dongle build inside your HMD (Index or Vive i assume) is handled exactly the same as the Vive Tracker dongles. It gets confused and pairs either a controller to a Tracker dongle or a Tracker to your HMD.
I have the problem myself.
To work around this you'll have to do the following in that order:
1.) Unplug all tracker dongles.
2.) Pair your Index controllers to your system
3.) Leave SteamVR running and unplug the USB from your Headset (Index) or turn off the Connect box (Vive). SteamVR will complain, but let it run anyway.
4.) Plug one Tracker dongle in, pair a Tracker to it and give it a role under "Manage Trackers"
5.) Unplug it
6.) Repeat 4. + 5. for all Trackers you have
7.) After you're done replug your Headsets USB or turn the Connect Box on again and restart SteamVR. Done.
It's a bit tedious, but you just have to do it one time for each Windows installation or if you wipe your SteamVR completely (e.g. for troubleshooting)
