Error 208 valve index
yesterday my headset was working fine, i played a game for 20 minutes, took a break, and when i went to continue i keep getting this error. i have unplugged everything, reset the usb ports for the hmd,updated my drivers, restarted my computer countless times. it seems most of the fixes for this issue were for the vive, not the index. I have contacted steam support, they answered once, none of what they said worked, but they haven't gotten back to me in 12 hours.

Same problem for me. Steam Support asked for various systemreports after i tried everything from the web/forum. No answer since then so far... in come cases a new connection cable seems to do the trick for some people.

Did your problem got fixed by now?
The following procedure was provided to me by Steam Support and I am happy to say that so far, it works! I consistently had error 103 (headset not detected).

1. Restart your computer and plug your Index into a USB 2.0 port.
2. Power cycle your Index; Remove the AC connection for 30 seconds.
3. Reset your SteamVR USB devices from Menu (3 lines) > Developer > Developer Settings > In the 'Reset' subsection, click 'Remove all SteamVR USB Devices'.
4. Disable Windows Enhanced Power Management for SteamVR devices from the same menu by clicking on "Disable Power Management."
5. Shut down SteamVR.
6. Restart your computer
7. Reconnect your VR equipment to USB 3.0+ ports, start SteamVR and retest the issue.

Thanks for your input. Didnt work for me tho. I think my problem is more adapter related :(

At the end removing the cable from the headset part and replugging it did the trick and the headset is working again :))

i tried for almost 2 hours trying to fix this, and what worked for me was going to my display settings and changing the resolution

Yes, direct mode is part of the steam support troubleshooting. I replacement worked fine for me too.

I fixed this on Windows 11. Zeke has it right. I checked the log and it only showed one display. In display settings I noticed only one display was enabled. I told windows to extend the monitor to both displays, then checked the logs and saw that the index wasn't the right resolution. When I changed the resolution of the Index SteamVR started working properly again!

I’m looked under the display settings for my headset, and couldn’t find it there, is there somewhere else I should look

Nothing has fixed this for me and I've tried everything but buying new parts and I don't see the reason to throw good money after bad. I've only had it work for 22 hours and now it's dead, so I don't think I'll ever be buying any steam hardware again.

Just make a Support Case if your device still has warranty. Steam Support sent me new hardware and its working fine since!
