Restart your computer and plug your Index into a USB 2.0 port.
Power cycle your Index; Remove the AC connection for 30 seconds.
Reset your SteamVR USB devices from Menu (3 lines) > Developer > Developer Settings > In the 'Reset' subsection, click 'Remove all SteamVR USB Devices'.
Shut down SteamVR.
Restart your computer
Reconnect your VR equipment to USB 3.0 ports.
Okay so, I know this solution is already out there but I HIGHLY recommend you look through the conflicting applications list. (just google steam vr conflicting applications)
I was using ASUS ARMOURY CRATE and that application seriously ♥♥♥♥♥ with VR for some reason. I uninstalled it and it is a LOT more stable. It still crashes in vr chat with 30+ people occasionally (again, NVIDIA and Valve need to update their ♥♥♥♥ to work with Windows 11) BUT it did seem to mitigate a lot of crashes.
Hope this helps and fingers crossed Valve and NVIDIA get with the times and update their software.
