I tried without success methods suggested here and elsewhere on the web: changing USB ports, removing the Steam VR configuration and reinstalling, changing the Windows 10 privacy settings... I also read that Asus motherboards had trouble with it, and since I have an Asus motherboard with an AMD Ryzen CPU, the last thing to try was to add a USB controller.
So in the end I purchased and installed a USB 3 controller (PEXUSB312A2) even though my motherboard has plenty of those. I switched the USB cable of the Valve Index onto it, rebooted, and the issue was solved, I can now use the camera functionality.
I also have a Asus Motherboard (Asus P9X79) and I'm hoping its not the issue but seemingly it is based on what everyone is saying. I'll give it a shot with a usb controller as well once I determine what to actually get and if I have the space on the motherboard. Suggestions appreciated.
Having the same issue here, you think an external usb 3 hub/controller would work aswell? Not really that comfortable installing hardware in my pc..
If you are paranoid, you can take care to discharge yourself from static electricity first, but just touching your computer case should take care of that. The card I used can optionaly be powered by an external power supply, but I dit not even bother to use it because the Index has it's own power supply.
If you have a laptop however, fat chance... You cannot add PCI cards on them.
I just plugged the USB-Cable into my PCIe USB Card (gladly, I have one already) and now it's working fine :)

i have exactly the same thing with the camera flicker when using windows camera, is that a hardware issue?
I resolved it by using a PCI-Express USB 3.0 controller by Innateck. Today ,I have a new motherboard that also works.
i have a VR ready 3.2 usb on my mobo... X470 GAMING PLUS MAX, that's so strange...
Edit: i changed the USB from the VR ready to a normal 3.2 and it works xD, and by works i mean no flicker, still have the communication failed error
Edit 2: for no reason at all today the camera works... don't know what to say, haven't done anything than changing the USB (that did nothing yesterday), not gonna complain, hope it keeps on working
Edit: Nevermind, I still get "Camera communication failed" from the camera test, but the "translucent ghost" camera feature works perfectly fine.

1. Go to Device Manager
2. Click the drop down arrow next to the Cameras section
3. Right click on eTronVideo and click properties
4. If its disabled by Windows, there should be an option to enable it on the
first window you see
After that, it worked perfectly for me
