Valve Index Controller "disconnecting"
I've had this issue where my right controller will "disconnect". The controller still has a green light, but SteamVR does not show it being connected (right controller is gray). I have tried re-pairing the controller in the same session, restarting SteamVR, and restarting Steam. It will not reconnect a strong majority of the time. It requires a system restart to work again. I might also add that if I plug it into the computer, the controller connects and is being completely tracked (although rumble doesn't seem to work). If I disconnect the controller from USB, it will show green light again and not track (or if controller was initially off, it will go back to being off). I have tried pairing the controller while plugged in and not plugged in, to no avail.

This problem usually occurs after an hour or so of use, sometimes more close to two or three hours. I have also plugged the controller in and disabled Windows power management under SteamVR settings as well as uninstall drivers of SteamVR devices plugged in via USB (followed by system restart) and reinstall. The problem still persists. I believe the only time I have endured this problem is in Bigscreen because that is the only application I have used for long periods of time (it is also my favorite I might add).

I am using 1.0 Base Stations with the Valve Index HMD and controllers. I have an i7 4790k, GTX 980 Ti, Gigabyte Gaming 7 z97 motherboard, and run on a Samsung SSD (I don't think these last few items matter but who knows).

My Valve index controllers also disconnect from time to time. Different games at random. I can reproduce by selecting panic at the disco on beat saber. That always makes it hiccup.

Yeah. Trying to play a game and the crap is annoying me.

I think it has to do with the USB hub in the headset itself. I remember an update for the headset seemed to fix it, but I had a similar issue not so long ago. I think I might've reinstalled the drivers for the USB hub in device manager.
