Index Both Controllers Will Not Connect At Same Time
I have the Valve Index and recently upgraded to a 13900k and 4090 from a 7700k, 1080ti. I am having this very weird issue where only 1 controller will connect at once.

If I turn off the connected controller, the other controller will connect, so this issue is not related to a specific controller. I have tried all potential options, including:

Disabling power management in Developer Options;
Uninstalling USBs Devices in Developer Options;
Hiding one controller to connect the other controller;
Uninstall Steam VR;
Use Beta Steam VR;
Unplug all USBs except Valve Index; and
Restart PC (several times).

Just to note, If I plug the non-connected controller into the PC via USB, both controllers will stay connected to the Index. However, if I unplug it from USB to PC, it will immediately disconnect.

I have already reached out to Steam Support. Any ideas? This seems to me to be some sort of issue with the Bluetooth hardware/software in the headset that should be able to be solved via factory resetting.

I appreciate any help. I am hoping to experience VR with the 4090.

You haven't mentioned re-pairing your controllers to your headset, have you tried this as part of one of your other troubleshooting attempts?

Yes, sorry. I have also tried re-pairing. I will re-pair controller A. Then to re-pair controller B, I need to turn off controller A. Both will successfully repair, but only one can be connected at the same time, if that makes sense.

Try deleting C:\Program Files (x86)\Steam\config\lighthouse (back it up first ofc)

It is where all of your steam VR settings are stored and it doesn't get deleted when you uninstall steam VR if it is like most other steam settings. You'll probably have to reinstall steamvr and then set everything up again but if it is a problem with a setting that might fix it. All I can think of though sorry.

I tried this, but unfortunately it was unsuccessful.

Steam Support had me recreate the issue on a different computer. I just set up my old PC (the 7700k, 1080ti), hooked up the VR, and have a fresh install of Windows 10, Steam, and Steam VR.

Still, only 1 controller will connect at the same time. I think this narrows it down to either the Index or the controllers as being the issue.

I am even more confused now because the Index worked prior to using the new PC. After attempting to boot up the Index through the new PC, this controller connect issue is now present with both the old and new PC.

Since it is common with both pcs then it seems like a factory reset is the way to go. I have seen instructions that a factory reset is just pressing and holding the menu button under the headset for 15s while plugging it in to power but while it is not connected to your pc, then connecting it to your PC and starting steamVR The issue is that I have was also given a similar instruction by valve's customer support to factory reset one of the knuckles grips and as far as I can tell it did nothing.

I am curious if you do end up trying a factory reset if it actually works.

I'm having the same problem, one of my friends says it's most likely an issue with software but I don't know, I tried everything you have (minus another PC) and still get no changes.

I'm thinking about factory resetting the headset as well but I've no idea if it'll work or just ♥♥♥♥ ♥♥♥♥ up more...

I just tried the factory reset. After plugging the headset in while holding the menu button, the Index rapidly blinks red. I then connected it to PC and started Steam VR. The headset continued to rapidly blind red without connecting to PC. I tried this a few times to see if I messed something up, but each time it was the same situation. It is entirely possible (more like probable) that I am missing something, but it seems that this is not working.

I followed those instructions, it asked me to install/repair the tracking upon startup and then I paired the controller that was stuck on blue and it worked! ^-^

It blinking red means that it can't start up and normally (from what I have seen, no first hand experience) this is fixed BY a reset and if a reset doesn't fix it then everyone I have seen says they RMA'd it.

The other thing I have seen cause a blinking red light on the headset is the cable (or the USB port the cable is plugged in to) being damaged.

Apparently the blinking red lights are used as a code for multiple types of failure (on the headsets, controllers and base stations too) and you tell by the blinking rates, but I don't know the specific blinking rates.

Have to say, the cables being damaged could also explain the entire issue, maybe as you were moving them around from the old PC to the new PC you bent a fragile part of the cable past its breaking point. But there's no convenient way to test that I am aware of. Breaks to the tether can cause a lot of oddities however.
