Steam VR crashing on load (Blocked Add-ons)
I have a PC fully capable of running steam vr. The vr i use is a vive cosmos elite which means that when i load up steam vr, vive setup also loads up, a couple seconds after all my add-ons will load up. My vive cosmos is fully set up and i dont think this is the issue. The add-ons i use which keep crashing are " SmoothTracking , Gamepad Support , htc_rr ". I'm not sure what the gamepad support is or htc_rr but they've never created an issue in the past.

Problem:
When clicking on the VR icon at the top left of the screen, everything starts to load up as normal. About 4 seconds into load up steam vr will encounter a critical error and crash. I do this process again and load up steam vr, this time it will load up fine but my add-ons will be blocked. I go into the steam vr settings and unblock them and restart steam vr, same error occurs.
Sometimes i can load steam vr without my add-ons installed and the only issue is that its the older layout of steam vr, and other times it will crash again which makes me believe that im having two issues at once or two different things are causing the same issue.

What i did to fix it: (nothing worked)
I've tried updating all of my drivers.
Updating my pc.
Deleting all steam vr files and uninstalling steam vr from the steam library multiple times.
Restarting/powering off my pc.
I've even tried checking all of my add-ons for updated versions and deleting and reinstalling. I've checked the integrity of my steam vr files. Same issues.
I saw a thread on Reddit saying that gamepad support was creating the issues and i followed the instructions on how to deleted the file, this didn't work.
I've ran steam as the administrator, no fix.

I think these problems started happening around October, it used to crash so i would reopen steam vr, unblock the add-ons, close steam vr, restart my pc and everything would be fine. Now i cant open steam vr at all and when i do managed to load it up, i cant use any of my add-ons which takes away an important part of my preferred gameplay.

The only thing left for me to do is uninstall steam from my pc and reinstall it. I don't want to do this yet as im not ready to reinstall all of my games. I did see a Reddit post saying i can keep all of my games but it was from 2 years ago.

If anyone has any information that could help, please let me know. I've been trying to fix these issues for a couple of weeks now and im about to give up.

there is not much that I can suggest since you seemed to do everything that I would have asked for you to try already. I would recommend to try and reinstall your headset drivers, run windows update, and even try repairing steam and steamvr. you could also try seeing if "sfc /scannow" and "dism /online /cleanup-image /restorehealth" in an elevated terminal fixes it.

also, if it is provided I would look up the steamvr error code and see if there is a solution to it. since I don't know exactly know what is happening, or have any sort of crash log or error code besides a common steamvr critical error dialog the best I can do is give basic advice that could solve the problem. also it might be worth a check to investigate the two other addons you have installed being the smoothtracking and the htc_rr and see if those are known to cause issues.
if all else fails after reinstalling the steam client and headset drivers then you may need to backup all of your files and nuke windows completely. (download the iso, flash a usb, run advanced setup and delete all partitions, let windows reinstall and resetup everything)
make sure you have a secondary pc or a flash drive with drivers already on them incase you need to manually install the network driver, once connected to the internet run windows update and go grab a coffee and some snacks while it downloads all the updates and other drivers needed then reinstall all of your apps and games and test if vr is working.
