USB device not recognised
I get this every time I load up Steam VR with a Valve Index:

USB device not recognised
The last USB device you connected
to this computer malfunctioned and
Windows does not recognise it.

I have Windows 10 (obviously). The error message only appears when I load Steam VR (I haven't seen it in Oculus). I only started getting this error when I changed from a 1080Ti GPU to a 3090.

If my computer is on and then I plug in and turn on my Valve Index I struggle to get a display even though the headset is still tracking and the controllers work. I have to keep turning it off and on again until I get a display. If I turn on the Index FIRST and then boot up my PC then it usually works straight away as soon as I load Steam VR. However the USB error message still appears.

I don't know how to fix it. Attempting to find and install a driver for the USB device that's got an exclamation mark against it in Device Manager Windows can't find a driver for it.

Is it possible your psu is not strong enough for everything with the 3090. What happens if you put the 1080ti back in.

depending on what other hardware you are running, you could need between a 750watt all the way to 1000watt.

if your running say a i5/ryzen5/i7/ryzen 7 then you need around a 750watt, and that is for a good fold standard, not a cheapy. If your runnin i9 or Ryzen 9 they recommend a 850wat. If your running Threadripper or intel equiv, they recommend around 1000watt.

Where as a 1080ti can happily use a 600watt for a lower end system to 750watt for higher end.

I have the same issue. It's definetly a software issue, same thing on 3 different computers. The 100% working method for me is first get the error then - developer options - remove all usb devices - restart pc - fast unplug index video/usb/power cords while PC restarts - start vr - plug everything in this order power/video/usb - Play. For me it usually happens if I unplug the headset while PC is running or keep it plugged in while it's booting up, the only prevention so far is to unplug everything while PC is off then plug in before playing. Weirdly it worked just fine when I bought it on my home computer and started acting up only after I switched the computers for the one in the garage since there is more space to play. And as I said also same error on a laptop, but resetting the USB works and I can play on all 3 computers just fine. I have not put that much time into it to actualy fix it but I hope I can find a better solution than removing the plugs everytime I want to play something while also not leaving the headset plugged in 24/7
